Overview
Arch pain (usually Plantar Fasciitis) is pain that is usually felt first thing in the morning or after rising up from a prolonged resting position and pain increases after more use of feet. The causes of arch pain include: weight, a foot injury, overuse, abnormal biomechanics of the foot (e.g. flat feet or abnormal pronation), footwear, certain activities, inappropriate training, and medical conditions (eg. rheumatoid arthritis). Treatment of arch pain include weight reduction, rest, ice, activity modification, anti-inflammatory products, sports massage, stretching, strengthening, acupuncture, strapping, steroid injection, ultrasound and orthotics.
Causes
The cause of this condition is too much pressure exerted on the arches, and although common in athletes, the condition can happen because you went hiking or climbing, you were lifting heavy objects, or you simply walked too far too vigorously. Pregnancy places extra strain on the arches because of both the additional body weight and the effect of hormones on muscles and ligaments.
Symptoms
Many people have no symptoms, and the condition is discovered only by chance when an X-ray of the foot is obtained for some other problem. When symptoms occur, there is usually foot pain that begins at the outside rear of the foot. The pain tends to spread upward to the outer ankle and to the outside portion of the lower leg. Symptoms usually start during a child's teenage years and are aggravated by playing sports or walking on uneven ground. In some cases, the condition is discovered when a child is evaluated for unusually frequent ankle sprains.
Diagnosis
The doctor will take a brief history to determine how the injury occurred. If necessary, a thorough physical exam may be conducted to evaluate for any other injuries. Taking your workout shoes to the exam may also provide valuable information to the medical practitioner. Both feet will be physically and visually examined by the medical practitioner. The foot and arch will be touched and manipulated possibly with a lot of pressure and inspected to identify obvious deformities, tender spots, or any differences in the bones of the foot and arch.
Non Surgical Treatment
If it is flat feet, then you'd seek professional advice and maybe need orthotics, or arch supports to prevent the pressures and to stop the pain. One of the other reasons you can get pain in this area of the foot is plantar fasciitis. The plantar fascia is a membrane that is inside of the skin and attaches to the heel bone here. It divides into three bands that go out of the foot here: the outer band, the central band, and the medial band here. Often, from impact, you get an inflammation of that attachment to the heel bone and this can often spread up the medial band and this is another way of getting pain in that arch. Now, the way to treat that is also using arch supports but also heel cushions, better soled shoes to prevent the pressure. These things normally disappear after a year, 18 months. Plantar fasciitis is easier to treat because it's not a long term problem. If you do need arch support, something like this would be very good for both problems.
Surgical Treatment
With flat feet, there is a tendon on the inside of the foot than can often become weakened, injured, split and/or ruptured. This tendon, is called the posterior tibial tendon, and is the main arch supporting tendon. Obviously damage to this tendon can cause collapse of the arch. Some people have genetically inefficient tendon, and tends to be the case in younger people. In mild cases, such as tendon splits, the posterior tibial tendon can be repaired to restore its strength. Acute incontinuity of the tendon can be primarily repaired. Often the posterior tibial tendon is augmented with a tendon transfer of an adjacent tendon to provide both strength and continuity. In any tendon repair, advanced or retensioning of the tendon is performed. In most flat foot surgery a tendon augmentation is often combined with other boney procedures to restore structure and balance to the foot.
Stretching Exercises
Ankle evert or strengthening. Lie on your side with your feet hanging off the end of your bed or a weight bench. Bend the toes of the foot that is closer to the ceiling slightly toward your head. This is the starting position. Now raise your toes toward the ceiling while keeping the rest of your leg stationary. Return to the starting position. Reps. 10-15. Now point your toes slightly away from your head. This is the starting position. Raise your toes toward the ceiling. Return to the starting position. Reps. 10-15. Ankle invertor strengthening. Same as above, but do the exercises with the foot that is closer to the floor. Dorsiflexor strengthening. Sit on a desk, table, or counter so that your feet don?t touch the ground. Let your feet dangle comfortably. Bend your foot upward as far as you can comfortably go. Do not let your foot pull inward or outward. Return to the starting position. Reps. 10-15.
